Hourly Home Care vs Live-In Home Care
Hourly Home Care
- Caregivers work set shifts such as 4, 8, or 12 hours
- One or more caregivers can cover the full 24 hours with shift changes
- Continuous alertness during the scheduled shift
- Often higher total cost for true 24-hour coverage because multiple shifts are billed
Live-In Home Care
- One caregiver lives in the home for a 24-hour period
- Caregiver provides daytime assistance and has a scheduled sleep time at night
- Fewer handoffs and strong relationship continuity
- Generally, more cost-efficient for round-the-clock presence if night needs are minimal
- Requires a private sleeping area and time for daily breaks
What Caregivers Help With
- Personal care such as bathing, dressing, grooming, toileting, and incontinence care
- Safe transfers and mobility using gait belts and approved techniques
- Medication reminders and routine cueing
- Meal preparation, hydration prompts, and light housekeeping
- Companionship, engagement, and gentle routines that reduce anxiety
- Support after hospital or rehab, including appointment coordination and equipment setup

FAQs: 24 Hour and Live-In Home Care
A live-in caregiver stays in the home for a 24-hour period with scheduled sleep time at night and reasonable breaks during the day. Families provide a private bedroom, access to a bathroom, and space for personal items. If nights require frequent assistance, an awake overnight shift is recommended instead of live-in.
Choose hourly coverage if there are multiple nightly wake-ups, higher fall risk, two-person transfers, active wandering, or complex routines that require an awake caregiver throughout the night. Choose live-in if nights are usually quiet and daytime help is the primary need.
Most 24-hour plans use 2 or 3 caregivers in 8 to 12-hour shifts. Each caregiver arrives alert and on duty, completes a handoff, and documents tasks, hydration, meals, mobility, mood, and safety notes.
They remain awake all night, assist with toileting and repositioning, provide cueing and reassurance, monitor wandering risks, and perform safety checks. This option is ideal after a hospital or rehab stay, during recovery, or when dementia symptoms increase at night.
A private sleeping space with a bed and linens, access to the kitchen and bathroom, and time for breaks as defined in the service agreement. Parking arrangements should be clear. Meals are either provided or a meal stipend is included according to agency policy.
Minimum shift lengths vary by area. Holiday rates may apply on major holidays. Changes or cancellations typically require notice to avoid fees. Your schedule can be adjusted as needs change after a quick review with the office.
Private pay is common. Many long-term care insurance policies reimburse non-medical home care. We can assist with documentation and claims. Medicare does not pay for non-medical custodial care. Medicaid and VA programs vary by eligibility and location. Contact us for a benefits check.
